Case #2:  This patient was referred to an orthodontist and later to a periodontal surgeon for corrective treatment for 4 congenitally missing teeth and crowding.  Towards the end of orthodontic treatment implants were placed, left to heal, and then provisional crowns were made to fill in the missing spaces before braces were removed.  After 1.5 years, the final crowns were placed.
